Registering with the surgery

You can register with the surgery in one of the following ways:

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

You will also be required to complete a GMS1 registration form which you can download and bring into the practice, or complete a copy at reception:

The practice manager will then call you in for a pre-registration interview.

The rules and regulations of the practice will be explained to you and you will have the opportunity to ask any questions to the practice manager.

Once your application has been accepted, you are then registered with the surgery and are booked in with our practice nurse for a new patient health check, whilst we wait for your medical records to arrive from your previous doctor.

Medical treatment is available from the date of registration. Please contact reception for further information or read our practice leaflet (PDF).
